Each district has its own district nurse together with a team of nurses and caretakers who provide home care.
This home care includes support with physical care, injections, wound care, providing medication, etc.
The district nurse indicates the necessary levels of nursing and care, thereby regulating access to care.
Together with the client’s team, general practitioner and many other collaborators, the district nurse tries to make sure that a client can continue to live safely at home for as long as possible. If this is no longer desirable/possible, we will look for a suitable form of housing together.
